Item level (ilvl) equals the area level where it dropped. The drop location determines how many sockets Larzuk gives, so check the class by area.

Use Larzuk for runeword bases. Cube recipe gives random sockets, but Larzuk guarantees max sockets on white/superior items.
Use Larzuk on unique/set/rare items. Only 1 socket opens, but cube recipe cannot socket these.
Larzuk in Normal is great when farming a 4-socket Crystal Sword for Spirit takes too long. Pick one up from Normal Cows — Larzuk guarantees 4 sockets.
For 2os armor bases like Stealth, skip Larzuk — just refresh Fara's shop in Act 2 to easily find one.
Repairing low-quality (Damaged/Crude) items via the Cube resets their ilvl to 1. Using Larzuk on these gives Class A (minimum) sockets only — avoid using Larzuk on repaired low-quality bases.
